No access to PIA support whatsoever

Ok, so chat was killed.  Now, the contact form does not work.  The fields for my name and email refuse to accept text.  The comments section of the form will accept text, however, without the name and email, the form refuses to allow submission.

How can I contact PIA to cancel?  I've forgotten my password over the years and my email has changed. 

PIA staff:  Please contact me with a phone number.  All I want to do is cancel.  I'm moving to a VPN service that has actual support.

    I was able to locate your support ticket and have responded there. I sincerely apologize for the delay and thank you for your patience!

    The form does work. It is just that certain fields have reduced font size and the color of the text is the same as the field background color. Hopefully PIA web gurus are working on this issue.

    I have exactly the same problem. This needs to be fixed ASAP since it's my only way of contacting support.
